When you incur high medical care costs

Your copayment for medical care costs is capped. If your copayment calculated based on certain standards exceeds the maximum, the excess amount will be paid as "High-Cost Medical Care Benefits".

When you want to reduce the burden of Medical Care Costs you have to pay over the counter (when you wish to have a Certificate of Application of Maximum Copayment Amount issued)

We recommend using a Myna health insurance card. By doing so, your cost-sharing maximum amount information will be provided with no need to give your consent and you will no longer need a Certificate of Application of Maximum Copayment Amount.

Required documents: Please apply through My Health Portal.
  • Log in to My Health Portal.
    If you have not yet registered for the first time and are unsure of your ID and PW, please contact

  • Click on the blue tile in the upper left corner that says "Application for Certificate of Application of Maximum Copayment Amount".
  • Click on "Application Form Entry" on the right.
  • Fill out the required information and click "Confirm entry".
  • Check that the information you have entered is correct and click "Submit".
  • The application is complete when you receive an email confirming receipt of your application titled "Application for Certificate of Application of Maximum Copayment Amount: Notice of Receipt of Application".
We will mail your certificate to the address you entered within 7 to 10 business days by Specified Recorded Mail. After discharge from the hospital or after the expiration date, please return the Certificate of Application of Maximum Copayment Amount to Sanofi Health Insurance Society.
Applies to: Insured persons or dependents whose copayment amounts for one month are expected to exceed their individual cost-sharing maximum amounts in the following cases:
  • Examinations and treatment received at a medical care institution or other facility that has not adopted the online eligibility verification system
  • When not using a Myna health insurance card
  • When not using a Myna health insurance card and you are 70 or older and in the same income category as active workers Ⅰ or Ⅱ
  • * Even when using a Myna health insurance card, those in the low income category must apply in advance for a Maximum Copayment/Reduced Standard Copayment Certificate to be treated as falling in the low income category.
